Car Seat Referral

In partnership with law enforcement, fire rescue, and community partners, Safe Kids Broward, led by Joe DiMaggio Children’s Hospital and Memorial Healthcare System's Trauma Department, has a program to assist identified families with child passenger safety needs or resources.

This form should be completed by the referring agency.
