To obtain a copy of a Medical Record from a Memorial Healthcare System facility, you must complete and submit an Authorization for Release of Confidential Medical Records Form. To download the form and learn more please visit the Medical Records section on our website.

How do I obtain a referral to a physician?

Visit the Physician Referral section on our website to learn more. Or, to find a Memorial doctor, call us toll-free at (800) 944-DOCS . To find your child's Joe DiMaggio Children's Hospital doctor, call us toll-free at (866) JDCH-DOC. Our Physician Referral Service is available 24 hours a day, 7 days a week.
