Memorial Healthcare System's Disease Management program helps uninsured members of our community manage chronic health conditions and links them to resources that enable them to live healthier lives. Experienced disease management nurses and eligibility counselors work with primary care physicians and other members of the healthcare team to deliver a personalized, coordinated approach to care.
Over the past 10 years, the program has served more than 9,500 chronically ill adults and children with such conditions as diabetes, asthma, hypertension, heart failure and HIV/AIDS.
The healthcare team helps ensure that patients follow their physician's plan of care to reduce the risk of complications, and they teach patients how to manage their condition at home. They also make sure the care that patients receive follows nationally recognized clinical guidelines for their condition.
